Public records requests must be made to the government agency that is in possession of those records. The purpose of the following form is to facilitate the release of public records which are held by the Oregon Department of Forestry. If the records you seek are held by another public body, you should instead request them from that body.

All administrative fees and costs related to document retrieval must be paid in full before records can be made available.

Please provide a detailed description of the records requested, including information that could narrow the search (such as dates and names). Limiting the scope of the request may reduce the cost of retrieving information.

Preferred Delivery Method Please note, requested information may not be available electronically.

I wish to receive electronic copies (e.g., CD, thumb-drive, e-mail).
I wish to receive printed copies via pickup or delivery.
